Dental pulp is the portion of the tooth that has blood vessels and nerves in it. Pulp is the core of the tooth. Pulpitis is an inflammation of the dental pulp and is the most common reason for toothache.

When the dental pulp gets irritated, the body responds by sending extra blood and defense cells to the pulp leading to an increase in volume of pulp. But the pulp doesn’t have the opportunity to expand as its surrounded by hard tooth structures like dentine and enamel.This increase in the pulpal pressure will be felt as pain by the patient.

The pulpal inflammation also makes the teeth more sensitive. For example, breathing cold air through the mouth, drinking hot drinks or chewing food can cause pain.

Pulpitis can be broadly classified as two : Reversible and Irreversible

Reversible Pulpitis

The symptoms of reversible pulpitis can range from nothing at all to a sharp pain on stimulation of tooth.

For instance, if you eat an ice cream,the cold causes a sharp pain in the tooth and it quickly goes away when you swallow. The good news is that reversible pulpitis, like its name, is reversible and it can go away if the cause of it is eliminated.

The causes of reversible pulpitis can be attrition of the tooth that reaches the dentin or a fracture of the enamel layer of the tooth which can expose the dentin or dental caries which has entered into deep layers of dentine etc.

Irreversible Pulpitis

The symptoms of irreversible pulpitis can range from no symptoms at all (eg:necrosed pulp) to an excruciating sudden pain. Post and Core : Things to know

A post and core is a type of dental restoration required when there is an inadequate amount of sound tooth tissue remaining to retain a conventional crown. In teeth which are severly damaged by caries or fracture, the clinician will recommend for an endodontic treatment followed by a clinical visit for post and core restoration. The decision of whether or not to place a post and what technique will be used to place it depend on the particular conditions of each case.

Pericoronitis : Infection Near Wisdom Tooth in Young Adults

Pericoronitis is a common problem in young adults when a tooth is not fully erupted into the mouth yet. The literal definition of pericoronitis is inflammation of the gum around the crown. This condition most of the time is very painful for the patient.

It usually occurs between 17 to 24 years of age, because that is when the third molars normally start erupting. But the doctor sometimes sees older patient with this clinical condition.

A clinical exam can help diagnose pericoronitis. Your dentist will examine your mouth and note your symptoms. The gums around the problem area are likely to be red, swollen or draining pus. Sometimes the inflammation is such that the patient can bite with the upper teeth, the inflamed gum of the lower jaw. The mouth opening may also be limited. An x-ray can also help determine if your third molars are erupting normally or not.

Pericoronitis is one of the common reasons for wisdom teeth removal, which an oral surgeon typically performs.

A piece of gum tissue appears over the biting surface of the tooth, which is very difficult to clean. Debris collects under the flap and, when the debris is not removed, the gum becomes inflamed. The tissue around a wisdom tooth has become infected because bacteria have invaded the area.

In cases of pericoronitis, food or plaque, which is a bacterial formation that remains on teeth after eating, may get caught underneath a flap of gum around the tooth. If it remains there, it can irritate the gum and lead to pericoronitis. If the pericoronitis is severe, the swelling and infection may extend beyond the jaw to the cheeks and neck.

Treatment for minor symptoms of pericoronitis (spontaneous pain, localized swelling, purulence/drainage, foul taste) is irrigation with specific liquid solutions and curettage of the area under local anesthesia. Major symptoms of pericoronitis (difficulty swallowing, enlarged lymph nodes, fever, limited mouth opening, and facial cellulitis/infection) are usually treated with antibiotics prior to the wisdom tooth removal.

Most of the time the symptoms will recur and the only definitive treatment is extraction. If left untreated, however, recurring infections are likely, and the infection can eventually spread to other areas of the mouth. The most severe cases may require intravenous antibiotics and surgery.

What are Wisdom Teeth?

Wisdom teeth or third molars are the teeth furthest away in the dental arches and they usually are the last ones to erupt, normally between age 18-22.

When they have not completed their eruption at that time and are completely submerged under the gum those teeth are called “impacted”, if they are only partially covered by the gum “semi-impacted”.

The extraction of third molars that are “symptomatic” is generally recommended, this means in cases where they cause pain, infections, damage to other teeth or if they are related with other pathologies like cysts and tumours.

The reason behind impaction of wisdom teeth, why some wisdom teeth are impacted, is not an such an easy question to answer. Generally a primary cause simply seems to be a condition of inadequate jawbone space behind a person’s second molar.

But why this lack of space exists is not yet fully understood.But there does seem to be a correlation between large tooth size and/or the presence of tooth crowding and having impacted wisdom teeth.

Pericoronitis

One of the most common clinical problems associated with third molars is “acute pericoronitis”. Its symptoms are sharp or throbbing pain, redness, swelling and/or purulence associated with the erupting tooth.

Additional symptoms may include limitation of mouth opening, dysphagia (difficulty to swallow), fever and lymphadenitis (swollen nodes). Pericoronitis occurs most of the times in molars that start erupting but remain partially capsuled by the bone or the gum, both of which retain bacteria preventing proper oral hygiene, resulting in inflammation and infection.

Damage to nearby teeth :

Decay

Decay can form on any tooth surface where dental plaque remains for extended periods of time. If the positioning of a wisdom tooth is such that it’s difficult to clean (which is frequently the case), it will be at increased risk of cavity formation in the adjacent teeth.

Even in cases where the decay is in the third molar and a filling could be successfully placed, if the wisdom tooth still cannot be adequately maintained and its surface kept plaque , it will run the risk of developing “recurrent decay”. This simply means that a new cavity has formed on an area of a tooth adjacent to an existing filling.

For the reasons above, once a cavity has formed on a wisdom tooth it’s generally advisable to simply extract it.

Root resorption

In some cases, depending on the position and angulation of “impacted” third molars, these become responsible for root resorptions (vanishing) of the adjacent second molars. The prognosis in these cases is bad and the patient may in the end lose more teeth.

Supra-eruption:

A tooth may supra erupt beyond the level of the neighbouring teeth if there is no supporting tooth opposing it. In this case the upper wisdom tooth supra erupts and erodes into the soft tissues of the mouth causing damage to the gum and infections.

Mal-erupt:

An impacted third molar may affect the normal eruption of an adjacent tooth. In some cases the permanent second molar’s eruption is adversely affected by the impacted third molar behind it, requiring the removal of both the third molar and the second molar.

Cyst and Tumors :

All teeth form within a sack and occasionally this sack can expand and grow resulting in a cyst formation around the wisdom tooth. Over time, this cyst can become larger and cause damage in the bone and other anatomical structures. The third molar also can be related in few cases with the formation of tumours in the jaws.

Recommendations :

It is very important to consider that every patient and every case are unique. The decision regarding removing a wisdom tooth or not must consider:
